How We Can Help

 Option 1: NDIS Application Overview – $25

(Approximately 20 minutes) 

This introductory session is perfect if you're starting to explore your options. We will  

  • Summarise the different pathways for applying for the NDIS  

  • Explain the application process in clear, simple language  

  • Highlight the types of evidence of disability the NDIS requires  

This session is designed to give you the confidence to proceed with your application, equipped with the essential information needed to move forward.

 

Option 2: Full Support for Your NDIS Application – $100  

(Approximately 1 hour) 

If you're ready to take the next step, this comprehensive session offers hands-on assistance. During this session, we will  

  • Guide you through the entire application process, whether you're applying via the Early Intervention Program or standard NDIS pathway  

  • Assist you in writing a strong application that accurately reflects your needs  

  • Provide detailed guidance on the supporting evidence needed, including what documents and reports to gather  

  • Answer any questions you have, to ensure you're fully prepared  

 

Ongoing support is available if you need further assistance, and we can negotiate packages to suit your needs.

Purpose of Support Coordination

Support Coordination helps NDIS participants:

  • Understand and implement their NDIS plan.

  • Connect with appropriate services and supports.

  • Build capacity and confidence to coordinate their own supports over time.

Key Responsibilities of a Support Coordinator

  • Understand the Plan:

    • Break down the NDIS plan and explain each funding category.

    • Clarify what supports can be used and how.

  • Connect with Services:

    • Identify and link participants with providers (e.g., therapy, health, housing, education).

    • Explore mainstream, community and informal supports.

  • Build Capacity:

    • Develop skills to manage supports independently.

    • Promote self-advocacy and informed decision-making.

  • Coordinate Supports:

    • Ensure all services are working cohesively toward goals.

    • Monitor budgets, address service gaps, and adapt plans if circumstances change.

  • Crisis and Change Management:

    • Respond to changes in circumstances or urgent needs.

    • Coordinate reviews or transitions when necessary.

  • Plan Reviews:

    • Prepare reports for NDIS plan reviews.

    • Assist with goal development and evidence gathering.

What’s Not Included

Support Coordinators do not:

  • Provide therapeutic or direct personal care services.

  • Make decisions on behalf of participants.

  • Manage finances (e.g. paying invoices – that’s a Plan Manager’s role).
